Learn more about Our School Change of School Request Form Due to our social distancing guidelines and school facility closures, we will be using on-line avenues for the 2020-2021 Request for Change in School of Attendance forms. There are two situations in which you would need to complete this form: 1. Your child currently opts into a school outside of your attendance area, and you plan to stay at the out-of-attendance-area school. 2. You would like to request your child go to a school for the first time that is outside of your attendance area.
